Challenges

Incident Reporting and Compliance

Capture incidents on site and report them the right way.

When an incident happens, the details matter and the clock is ticking. We give staff an app to capture an incident on the spot, then automate the report to your governance team and, where needed, to government.

The problem

Why is incident reporting so hard to get right?

Incidents happen away from a desk, often under stress. Paper forms get filled in late, miss detail, and take days to reach the people who must act and report.

Capture the incident as it happens

Incidents recorded on site, as they happen, with the detail intact.

Send it to governance at once

Reports reach your governance team straight away.

Report to government when required

The right details ready for regulator reporting when needed.

The impact

What poor incident reporting costs you

A weak incident process carries real risk. Missed or late reports can breach the rules you must follow. Thin records make investigations harder. The same causes repeat because no one sees the pattern. And leaders carry risk they cannot see or prove they managed.

Missed or late reports that breach the rules

Late or missed reports that put you on the wrong side of the rules.

Thin records that slow investigations

Patchy detail that drags out every investigation.

The same causes repeating over time

Root causes recur because the record is too thin to learn from.

How we help

We capture the incident and automate the report

We build an app that captures an incident in full at the point it happens, then automates the workflow. The right people are told at once, the record is complete, and government reporting happens when the rules require it.

01

Capture in full

Staff capture the incident on a phone, with photos, witness notes and the location, while the detail is fresh.

02

Notify the right people

The report goes to your governance team at once, with a clear owner, so nothing waits in an inbox.

03

Meet the rules

We build in the steps your rules require, including reports to government, so deadlines are not missed.

04

Learn from it

A dashboard tracks incidents and causes over time, so you can act on patterns and prevent the next one.

The result: incidents reported in full and on time, with a record you can stand behind.

Customer Spotlight

How this works in practice

A childcare provider needed staff on site to capture incidents quickly and report them to internal governance, and to government when required. We built an app that captured the incident in full on a phone, then automated the workflow so the right people were told at once and nothing was missed.

The outcome

Incidents captured in full on the spot

Governance told the moment it happens

Government reporting handled by the rules

A clear trail for every incident

Trust and governance

Safe by design

AI is only useful if you can trust it. We build automation and agents you can stand behind in an internal review or in front of a regulator. We design for Australian data and privacy expectations, which matters most in government, health, education and financial services. Your data is never used to train public models.

Controls we build in

  • Identity controls
  • Least-privilege access
  • Clear data-handling rules
  • Audit logs
  • Human handoffs

Questions we hear a lot

Common questions about incident reporting

What does an incident reporting app do?

It lets staff capture an incident where it happens, with photos, witness notes and the location. The report then moves on its own to your governance team, and to government where the rules require it, so nothing is late or missed.

Can it handle government reporting?

Yes. We build the steps your rules require into the workflow, so a report that must go to government is prepared and sent on time, with a record that it was done.

Does it work offline?

Yes. Staff can capture an incident with no signal, and the app sends it as soon as the device is back online, so a poor connection never stops a report.

How is this different from a paper form?

A paper form is slow, easy to skip and often filled in late from memory. The app captures the detail on the spot, routes it for you, and keeps a complete, time stamped record.

Is it secure and compliant?

Yes. It runs in your Microsoft environment, with access controls and audit logs, so sensitive detail is protected and you can show who saw what and when.

How soon after an incident should it be logged?

As close to the moment as possible, because detail fades quickly. That is the main reason to move off paper. The report gets written while people still remember it.

Can we still capture an incident if someone is not on the app?

Yes. We keep a way for a supervisor to log on someone behalf, because the person involved is not always the right person to be filling in a form.

Who gets told, and when?

You set the rules. Typically the supervisor and safety team immediately, with anything serious escalating on its own so it does not depend on someone remembering.
